What is ASC surgery?

ASC is a surgical treatment for scoliosis that does not involve planned fusion of the vertebrae. Through an anterolateral approach, anchors are placed in the vertebral bodies and connected with a flexible cord. The technique corrects the deformity while preserving spinal mobility to a far greater extent than fusion surgery does. The outcome depends on the type, magnitude and flexibility of the curve.

How does ASC differ from spinal fusion?

In fusion surgery the vertebrae within the corrected segment are held by a rigid construct so that they knit together. ASC uses a flexible cord and does not aim to fuse the instrumented vertebrae, which is what allows mobility to be preserved. For some patients, however, fusion may remain the more appropriate or the safer option.

How does ASC differ from VBT?

ASC and VBT overlap, because both rely on anterior vertebral tethering with a flexible cord and neither involves fusion. VBT is mainly associated with gradual correction achieved through the child’s remaining growth. ASC places its main emphasis on correcting the deformity during the operation itself and may be considered in carefully selected adolescents and adults.

Who may be a candidate for ASC surgery?

ASC may be considered in adolescents and adults with a sufficiently flexible scoliotic curve that requires surgical treatment. The assessment takes into account curve type and flexibility, the Cobb angle, trunk balance, bone quality, skeletal maturity, previous surgery and general health. The indication is established by the surgeon after reviewing the radiographs and completing a full assessment.

Who performs ASC surgery?

ASC operations are performed personally by Prof. Iakiv Fishchenko together with a dedicated surgical team at the Institute of Traumatology and Orthopedics of the NAMS of Ukraine. Consultations take place at 27 Bulvarno-Kudriavska Street, office 702, Kyiv.

How much ASC experience does Prof. Iakiv Fishchenko have?

Prof. Iakiv Fishchenko has been performing ASC surgery personally since 2021. His personal experience comprises more than 150 such operations in adolescents and adults. This figure describes the surgeon’s practical experience; it does not guarantee any particular outcome for an individual patient.

Which radiographs are needed for an online consultation?

For an initial assessment, please send standing full-spine radiographs in the anteroposterior and lateral projections via Telegram. After reviewing them the surgeon may additionally request bending films, MRI, CT or other investigations. The final indication is established only after a complete assessment.

What results can be expected, and what are the risks of ASC?

The goal of ASC is to reduce the curve, vertebral rotation and trunk asymmetry while preserving spinal mobility as far as possible. The degree of correction and the course of recovery are individual. Possible risks include infection, bleeding, injury to neural, vascular or thoracic structures, problems with the implants or the cord, loss of correction, progression of the deformity, and the need for revision surgery or fusion.

What does recovery after ASC involve?

After surgery the patient is mobilised gradually under the supervision of the team. The length of the hospital stay and the time before returning to school, work and sport depend on the extent of the operation, the patient’s age and the course of recovery. An individual loading programme is set by the surgeon and the rehabilitation specialist.
